To join HI360, you need to be: 1) 18 years or older, 2) able to speak English or Spanish, and 3) able to take part in team activities. We hope to have about 330 people participate over a few years!

You can enroll with up to two other adults!

We are not enrolling children in HI360 but you will be allowed to bring your child(ren) to the sessions. They are family friendly!

HI360 is taking place in Detroit and Flint. Right now, HI360 is only running in Detroit at the Detroit Hispanic Development Corporation (DHDC), Eastside Community Network (ECN) and the Community Health and Social Services Center (CHASS). Once we get HI360 started in Detroit, we’ll start working with community partners in Flint to get it going there.

Everyone who enrolls in HI360 will be randomly placed into one of two waves. One wave will start the program right away, and the other wave will wait until the first wave is done to start.

1. Attend a pre-program data collection event where you:

   1. 1. 1. Sign an agreement to participate in HI360,

         2. Have your height, weight, blood pressure, cholesterol, and blood sugar measured,

         3. Complete survey questions about your lifestyle and health

2. Participant in the 20- week (about 5 months) HI360 program with 10-15 other adults, now or in a few months:

   1. 1. 1. Two sessions per week for the first 8 weeks

         2. One session per week for the last 12 weeks

         3. Each session is about 2 hours

         4. Each session will have team walking, health education discussions, and/or hands on cooking focused on improving your overall health.

3. Attending a post-program data collection event where you will:

   1. 1. 1. Have your height, weight, blood pressure, cholesterol, and blood sugar measured,

         2. Complete survey questions about your lifestyle and health

Risks: Joining HI360 may have some minimal risks or discomfort. For example, a quick finger prick will be done to check your blood sugar (hemoglobin A1c) and cholesterol levels. You might also feel some emotional discomfort when answering certain survey questions, like those about mental health, or when being weighed.

Overall, the activities and data collection in HI360 shouldn't be any riskier than things you do in daily life.

Benefits: Taking part in HI360 could lead to improvements in your overall health and well-being, including a lower risk of heart disease, but there is no promise that this will happen.

It's both! This program was created by working together with different community-based organizations to help improve health in your area. These organizations will run the program as part of their other health and wellness activities. The University of Michigan, in partnership with the community-based organizations, will study if the program helps improve the health of those who participate.

1. The HI360 could take up to 30 hours over about 20 weeks/5 months:

   1. 1. One hour for the pre-program data collection event

      2. 28 hours for the HI360 program (over 20 weeks)

         1. 1. Two sessions for the first 8 weeks (4 hours)

            2. One session for the last 12 weeks  (2 hours)

In order to start everyone strong, we created the program to have 2 sessions during the first 8 weeks and 1 session for the last 12 weeks. We know it might be hard to attend all the sessions,  but your team leader will work with you and your team members to schedule sessions at times that work for most, or all, of your group.

Yes! We know your time is important, and we appreciate you spending it with us. You can get up to $70 for participating in the data collection events. You’ll get a $35 gift card two times—once at the beginning and once at the end for taking part in data collection events.

More importantly, you may also see improvement in your overall health, though there is no guarantee. We hope that you’ll enjoy spending time with others who are working on similar goals, getting to know other folks in your community, and learning about resources to help keep you going on your health journey!

You are able to change your mind at any time! If you decide to join now, and change your mind later, just let your team leader or another HI360 staff member know. There is no penalty for changing your mind.  

Other studies tell us that it can take this much time to help people change their lifestyle habits for the better. We want to make sure this program is good for you and helps you make lasting improvements in your overall health.

Yes. You have to be a part of the study to participate in the program. 

Because HI360 is a new program that takes the best parts of other successful health programs and puts them together, we need to test it first to see if it works well before offering it to others. Because we will be testing the program, it becomes a study.

Joining the study and participating in the program is your choice, it is voluntary, but only people in the study can try the program right now. If you decide to join, you’ll be asked to go to 2 data collection events and participate in the program activities. We’ll use the information we collect to see if the program is helping, and we’ll share what we find with you once the study is over.

If you decide to join, you can stop at any time. If you decide to stop, just let your team leader or a member of the study team know.

If you decide to join, you’ll be invited to a health fair-like event at the start and end of the program to collect health information about you. We will use this information to help determine if the program was helpful in improving your overall health. 

• There will be different stations set up where you will be asked to give different health information. These stations will include:

  • • Permission to Participate (Informed Consent),

    • Height and weight,

    • Blood pressure,

    • Blood sugar and cholesterol (finger prick),

    • Surveys: You’ll be given a table, computer, or paper to answer questions about your health, like what you usually eat and how much sleep you get.

• These events will be at one of your local community organizations

• They will take about an hour.

Informed Consent is a process when you get a full explanation of all HI360 activities, including risks and benefits, and decide formally whether you want to participate. We’ll go over a document that explains what will happen to you and your information during and after HI360.  You’ll be given a chance to ask questions and time to think about joining HI360. If you decide to join, you will sign a consent form. This form documents that you understand what HI360 is about and agree to participate.

A group! Groups will be about 10-15 people. This is an important part of HI360. By participating in a group, you get support from one another and build relationships with other members of your community. Each group will be led and organized by a community health worker/group facilitator and a group leader. 

The group facilitators are all community health workers committed to the success of the groups at each HI360 site. They will organize and lead group sessions. They will be your resource!  They are also part of the HI360 study team, assisting in data collection and monitoring of HI360 activities.  

The group leader is someone you pick!  They are part of your group and will help support you and the group facilitator. For example, the group leader may be in touch with other group members to provide extra encouragement, or to follow up if anyone has to miss a session. The group leader will also be a connection between you and HI360 leadership, including the facilitator, to address any group concerns or share ideas for tailoring sessions based on group interests.

HI360 was created to improve overall health, including your physical health (e.g., your blood pressure, cholesterol, and blood sugar), as well as your mental health, and social health.

Each session will include walking together and generally focus on 1 topic. These topics include things like:

• Hands-on food and nutrition, where you make meals together to enjoy as a team,

• Sessions where you discuss a specific health topic, such as getting enough sleep,

• Sessions on community and policy advocacy,

• Talk as a team about how to overcome certain challenges to improving your routines and habits, given how hard it can be to take care of ourselves each day.

• Your community health worker may bring in guests from other community organizations or agencies to help with these discussions,

• Community resources will be shared so everyone knows what is available to them. 

You’ll get a detailed list of topics and schedule when the program starts.

Yes! Sessions are family friendly. Each session will include a walking part and a discussion part.

Yes! You can be placed in the same team with up to 2 other people. Just let us know when you enroll who those other people are.
